CUNY Student Forms

Starting with the 2022-2023 financial aid award year, students can submit their 2022-2023 Verification and most FAFSA-related documentation using the new platform called CUNY Student Forms. The CUNY Student Forms platform provides a secure portal for FAFSA-related document submission and electronic signature, making document submission faster and easier for students with the device of their choice (phone, tablet, or computer).

Students can access CUNY Students Forms by using their existing CUNYfirst login credentials. To access tutorials and learn more about CUNY Student Forms, visit the CUNY Student Forms FAQ.

First-time users will be required to create their account.

How to create your account:

Step 1: Log into your CUNYfirst Student Center at home.cunyfirst.cuny.edu.

Step 2: If you are required to submit FAFSA-related documentation, you will see “Verify My FAFSA” listed on your To-Do List.

Note: If you do not see “Verify My FAFSA” listed on your To-Do List, you do not have FAFSA-related task(s) to complete on CUNY Student Forms.

Step 3: Click on the “Verify My FAFSA” item detail link.

Step 4: Complete your two-step security confirmation by e-mail or SMS. You must provide a valid email address or phone number where you will receive a secure code.

Step 5: Verify that your first name, last name, date of birth, and social security number match exactly as reported on your FAFSA.

Once you have successfully created your account, you can view and complete your FAFSA tasks electronically on CUNY Student Forms.

Note: Students who do not have FAFSA related tasks to complete will not be able to access CUNY Student Forms since no action is required from the student for Verification or FAFSA related tasks on CUNY Students Forms.

CUNYfirst Secure Document Upload

For financial aid document submission for the 2021-2022 financial aid award year, students can securely upload their documents using the “Document Upload” feature available in CUNYfirst Student Self-Service. Note: Do not email documents containing Personally Identifiable Information (such as a social security number, date of birth…).

For best results, please download the forms and fill them out using Adobe Acrobat or comparable software.
